Is There an Oral GLP-1?
The brief
- Three oral GLP-1 products are on DailyMed: Wegovy tablets and Foundayo for weight, and Rybelsus for type 2 diabetes.123
- In OASIS 4 (307 adults), oral semaglutide 25 mg gave a mean weight change of -13.6% versus -2.2% on placebo at 64 weeks.4
- In ATTAIN-1 (3,127 adults), orforglipron 36 mg gave -11.2% versus -2.1% on placebo at 72 weeks.5
- The semaglutide tablets require a morning fast and a 30-minute wait; Foundayo can be taken with or without food.12
The short version: yes, and they are not interchangeable. The labels, the trials and the dosing rules differ.
Yes. As of this writing there are three FDA-approved oral GLP-1 receptor agonist products: Wegovy tablets (semaglutide) and Foundayo (orforglipron) for weight management, and Rybelsus (oral semaglutide) for type 2 diabetes.123 They differ in how they are taken, what they are approved for and how much weight was lost in their trials, and no head-to-head obesity trial of the two weight-loss pills was found in the sources reviewed for this article.
Which oral GLP-1 medications are approved?
| Wegovy tablets | Foundayo | Rybelsus | |
|---|---|---|---|
| Active drug | Semaglutide (peptide) | Orforglipron (small molecule) | Semaglutide (peptide) |
| Labeled use | Weight reduction in adults; lower risk of major cardiovascular events1 | Weight reduction and long-term weight maintenance in adults with obesity, or overweight plus a weight-related condition2 | Blood sugar control in adults with type 2 diabetes; lower cardiovascular risk in those at high risk3 |
| How it is taken | Empty stomach in the morning with up to 4 ounces of plain water; wait at least 30 minutes before eating, drinking or taking other oral medicines1 | Once daily with or without food; swallow whole2 | Same fasting rules as Wegovy tablets3 |
| Starting dose | 1.5 mg daily for 30 days, then stepped up every 30 days1 | 0.8 mg daily for at least 30 days, then stepped up2 | 3 mg daily for 30 days, a dose the label says is not intended for blood sugar control3 |
Wegovy tablets share a label with Wegovy injection, and Rybelsus shares one with Ozempic tablets.13 Compounded or unapproved "oral" versions sold online are a separate matter and are not covered by these labels.
How much weight did people lose on the pills?
Each trial below was funded by the drug's manufacturer, and the trials used different populations, lengths and doses, so the numbers should not be lined up as a ranking.
| Trial | Drug and dose | Design | Mean weight change vs placebo |
|---|---|---|---|
| OASIS 44 | Oral semaglutide 25 mg | 307 adults without diabetes, 2:1 randomization, primary end point at 64 weeks | -13.6% vs -2.2% |
| OASIS 16 | Oral semaglutide 50 mg | 667 adults without diabetes, 68 weeks | -15.1% vs -2.4% |
| ATTAIN-15 | Orforglipron 6, 12 or 36 mg | 3,127 adults with obesity, no diabetes, 72 weeks | -7.5%, -8.4% and -11.2% vs -2.1% |
In OASIS 4, the semaglutide group was significantly more likely than placebo to reach weight reductions of 5%, 10%, 15% and 20% or more.4 In ATTAIN-1, 54.6% of the 36 mg group lost 10% or more of body weight, against 12.9% on placebo, and 18.4% lost 20% or more, against 2.8%.5 OASIS 1 tested a 50 mg dose; the 25 mg dose is the one in the Wegovy tablet label.61
One caution on reading doses: the Foundayo label lists tablet strengths from 0.8 mg to 17.2 mg, which are not the same numbers as the 6, 12 and 36 mg doses in the trial, so trial milligrams should not be read as label doses.25
How do the pills compare with each other?
The closest direct comparison is in type 2 diabetes, not obesity. ACHIEVE-3 was a 52-week, open-label trial of 1,698 adults with type 2 diabetes on metformin, comparing orforglipron with oral semaglutide at 7 and 14 mg. Hemoglobin A1c fell by 1.91% on orforglipron 36 mg versus 1.47% on semaglutide 14 mg, and orforglipron met the trial's non-inferiority and superiority tests.7 The same trial found more stomach and intestinal side effects (58% to 59% versus 37% to 45%), more stopping because of side effects (9% to 10% versus 4% to 5%) and a larger rise in pulse (3.7 to 4.7 beats per minute versus 1.0 to 1.5) with orforglipron.7 It was sponsored by the maker of orforglipron and open-label, which means participants and investigators knew which drug was being given.
For weight loss, the two pills have been tested in separate trials, and indirect statistical comparisons exist but rest on assumptions that a randomized head-to-head trial would not need. Anyone comparing them should treat claims that one pill "beats" the other with caution.
What are the side effects and warnings?
All three labels carry a boxed warning about thyroid C-cell tumors. Semaglutide causes these tumors in rodents; for orforglipron the label notes it is not pharmacologically active in rats and mice and did not produce tumors in rodents, and that the relevance to humans has not been determined for either drug.123 Each is contraindicated in people with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2.123
The most common side effects are gastrointestinal. The Foundayo label lists nausea, constipation, diarrhea, vomiting, dyspepsia and abdominal pain among reactions reported by 5% or more of patients, along with headache, fatigue and hair loss.2 The Wegovy label lists a similar set.1 In OASIS 4, gastrointestinal adverse events occurred in 74.0% of the semaglutide group and 42.2% of the placebo group.4 In ATTAIN-1, adverse events led to stopping treatment in 5.3% to 10.3% of orforglipron groups, against 2.7% on placebo.5 Our overview of GLP-1 side effects covers the wider class.
Interactions are worth asking about. Because the semaglutide pills must be taken with a small amount of water and nothing else for 30 minutes, timing against other oral medicines matters.1 The Foundayo label advises people on oral hormonal contraceptives to switch to a non-oral method or add a barrier method for 30 days after starting and after each dose increase, and says to avoid strong CYP3A4 inducers.2 Both labels advise against using another GLP-1 receptor agonist at the same time.12
Pill or injection?
The pill is a choice about routine and tolerance, not a different kind of drug. Semaglutide in tablet form is the same molecule as in the weekly injection, and Wegovy injection and tablets are covered by one label.1 Which format suits a person depends on whether they can manage a strict morning fasting routine, whether they dislike injections and what a prescriber considers appropriate for their health. Neither label suggests a pill is a way around dose escalation: all three start low and step up over weeks to limit gastrointestinal effects.123 For a look at an injectable alternative see what tirzepatide is, and for how weight loss affects lean mass see GLP-1 and muscle loss.
What this means for someone considering one
Oral GLP-1 medicines are real, approved and prescription-only. The trial data show clear average weight loss compared with placebo, alongside frequent gastrointestinal side effects. They are not suitable for everyone, and the decision on whether to use one, and which, belongs with a prescriber who knows the person's medical history and other medicines.
